Intuitive Surgical Trades at 44x Earnings Amid Slowing Growth

ISRG stock faces a 24% upside target against a 5% downside risk, yet recent price history shows limited success in this range.
Key points
- ISRG trades at 44.4x earnings, double the S&P 500 median, despite slowing procedure growth.
- US procedure growth slowed to 12% in Q2 2026, down from 14% in Q1.
- A new instrument reuse program starting in 2027 will lower revenue per procedure.
Intuitive Surgical (ISRG) currently trades at 44.4 times earnings, nearly double the S&P 500 median of 22.4. This valuation premium relies on continued expansion, but recent operational data suggests momentum is cooling. The company’s stock sits at a technical level where historical rebounds have frequently stalled before achieving significant gains.
According to Yahoo Finance analysis, a move to the April high of $487.45 represents a 23.9% gain from the current price of $393.33. The downside risk is limited to a 5% drop to $373.66. However, over the last two years, the stock has entered this price range four times without producing a 20% gain before a 5% decline, indicating that the risk-reward ratio is not supported by recent price behavior.
Procedure growth slows in key markets
US da Vinci procedure growth decelerated to 12% in the second quarter of 2026, down from 14% in the first quarter. The slowdown is concentrated in elective procedures that patients can delay. Management cited coverage changes and shifting patient timing as primary factors, noting that some customers report these external pressures affecting care scheduling.
Pricing pressure hits revenue per procedure
Starting in the first half of 2027, an extended use program will allow certain instruments to be used for more procedures. This change lowers the cost per procedure for benign cases, directly reducing revenue per unit. While Intuitive Surgical expects this to widen adoption, the shift poses a challenge for a stock priced for high growth, as the revenue per procedure metric is expected to decline.
System placements sustain overall revenue growth
Despite procedural slowdowns, Intuitive Surgical placed 468 da Vinci systems in Q2 2026, an 18% year-over-year increase. However, approximately half of US placements were trade-ins from existing customers upgrading their systems. Total revenue grew 20.7% over the last twelve months, significantly outpacing the S&P 500 median of 8.3%, though the mix of new installations versus upgrades is a critical factor for future expansion.
